Closed Preservation Rhinoplasty in the Mestizo Patient: Challenges and Techniques for Nasal Tip Support

Overview

Mestizo noses have thick skin and weak tip cartilage, so most surgeons use open structural surgery for the tip. Valdivia, Durand and Cakir (Cakir is a leading figure in closed preservation surgery) retrospectively reviewed 417 primary rhinoplasties from January 2022 to December 2023, all closed with dorsal and soft-tissue preservation, and describe four tip techniques used according to anatomy.

• 417 patients: 331 women (79.3%), 86 men (20.6%); ages 15-66, median 23.
• Tip technique distribution: Type I 105, Type II 124, Type III 181, Type IV 7.
• Follow-up 6 months to 1 year.
• 7 revisions (1.7%).
