Two-sentence lead. Bleeding, bruising, swelling, infection and graft problems are the main surgical rhinoplasty complications; infection runs 0.62% to 0.84% in series of 2,630 and 3,084 cases and 19-fold higher in revisions, tranexamic acid, desmopressin, piezoelectric bone cutting, drains and antibiotic soaks each reduce a measured complication, while nasal packing and subperiosteal tunnels add little. Filler rhinoplasty carries a separate risk of arterial occlusion in about 0.27% of patients, with blindness, necrosis, and stroke reported, and every published necrosis case in one series followed a prior rhinoplasty.
