What the literature says
Synthesised from the abstracts of the 40 papers listed under Start here. Every line carries the PubMed ID it rests on.
Two-sentence lead. Filler rhinoplasty produces satisfaction of 92-99% in pooled series with mostly mild side effects, but about 0.2-0.3% of patients suffer arterial occlusion, skin necrosis ran 0.06% in 5,000 primary treatments and 0.47% in 2,088 post-surgical corrections, and vision loss when it happens is usually immediate and permanent. Risk is higher after prior surgical rhinoplasty, the bridge is the most common injection site in reported blindness cases, effect lasts 6 to 14 months, and 74% of surveyed European surgeons now operate on noses that carry filler.

What is still contested or missing
- Complication rates range from 7.6% [33270519] to 39.11% [38862661] depending on whether erythema and swelling are counted [33270519, 38862661]
- HA rheological properties were not accounted for in the pooled outcome data [37783865]
- The filler-lymphatic edema mechanism is a hypothesis without objective measurement [39532285]
- Well-designed RCTs are still lacking [37217605]; commentaries [38977451, 39466798, 38923925, 33270498, 34468422] are title-only on file [37217605, 38977451, 39466798, 38923925, 33270498, 34468422]
- Vascular events: 0 in 1,600 pooled patients and in three protocol series versus 0.27% severe occlusion in 9,657 pooled patients and 22 published blindness cases; under-reporting is suspected by the reviewers themselves [32590640, 38862661, 42660550]
- Whether nonsurgical revision should come before surgical revision is a single injector's claim from 2,088 cases [39247575]
- Filler effect on nasal cartilage shown only in 16 rabbits at 90 days [39965763]
- No consensus among 165 surgeons on dissolving, timing or removing filler before rhinoplasty [39993423]
- Injector-experience data are 59 patients across 53 injectors [41813985]
