Functional rhinoplasty that corrects the septum and nasal valves lowers obstruction scores by roughly 40-50 NOSE points, and the gain holds at 12 months; endonasal septal extension grafting alone cut NOSE by 49.4 points in a 34-patient pilot. Preservation and structural techniques breathe equally well (randomized trial of 84 patients: SCHNOS P=.90), spreader graft versus spreader flap shows no measured functional difference, and NOSE and SCHNOS-O scores can now be converted with a validated crosswalk.
