Removing the hump and preserving the dorsum give the same long-term result; preservation shows a small cosmetic edge in the first 6 months and fewer late complications in pooled data, a randomized trial of 84 patients found no difference at 1 year, and blinded raters picked the technique correctly only 52.9% of the time. Piezoelectric (ultrasonic) bone cutting reduces early swelling, bruising, and pain compared with conventional osteotomes without changing osteotomy time; hump recurrence after preservation runs 2.5% to 13.6% depending on how the cartilage is contoured and fixed.
